What’s Included in Your Subscription

$99/mo for the account holder, plus $29/mo per additional family member. Here’s exactly what you get.

Per submission

Every submission you make on your plan includes:

  • A personal physician response. A board-certified U.S. physician reviews your full letter — your symptoms, history, and what your AI told you — and writes back with a clinical assessment, personalized guidance, and a prescription if medically appropriate.
  • Knowledge-base-backed review. Our reviewer draws on a curated library of medical literature (American Family Physician, Cochrane reviews, and others) to inform their decision.
  • Image attachments. Up to 3 photos per submission — rashes, wounds, test results — for the physician to review.
  • E-prescription to any U.S. pharmacy. When a prescription is written, you receive a text message to choose your pharmacy.
  • Async clinician chat. If the physician needs more information, they’ll message you in your encounter — and you (or your AI agent) can reply.

Per account

  • Unlimited encounters. Submit as many as you need, subject to our Fair Use Policy.
  • Family members on one plan. Add adults (spouse, parents) and kids as young as 6 months at $29/mo each.
  • Searchable encounter history. Every submission and physician response stays in your account.
  • Use any AI agent. Claude, ChatGPT, Gemini, or any agent that can follow instructions can compose and submit on your behalf.

Per family member (seat)

Each additional family member is +$29/mo and includes everything above — their own encounters, their own physician responses, their own pharmacy preference. Each adult family member completes identity verification once; minors inherit the account holder’s verification.

What’s not included

  • In-person care. Appendix is a telehealth practice. If a physical exam is required, the physician will direct you to in-person care.
  • Controlled substances. We do not prescribe controlled substances under any circumstances.
  • Mental health crisis care. For crisis support, call or text the 988 Suicide & Crisis Lifeline.
  • Emergency care. For medical emergencies, call 911 or go to your nearest emergency department.
  • Insurance billing. Appendix is cash-pay only. We do not bill insurance.
