Common questions about Appendix, prescriptions, and how the platform works.

Is an AI writing my prescription?

No. AI helps gather and organize your medical information, but every prescription decision is made by a licensed, board-certified physician who personally reviews your submission. The AI never makes clinical determinations. Appendix provides a higher level of physician oversight than a typical health conversation with an AI, because an actual board-certified physician is involved and personally reviews everything before any prescription is issued.

Do I need an API key?

No. All public endpoints (agent query, knowledge search, conditions, pricing) are freely accessible without authentication. Rate limits apply per session (5 queries/hour, 10/day). If you need higher limits for production use, contact hello@appendix.com.

Which states is Appendix available in?

Appendix's Knowledge Base is available worldwide for free. Clinical services (physician review and prescriptions) are available by paid subscription in 18 U.S. states: Alaska, Arizona, California, Colorado, Connecticut, Florida, Illinois, Kansas, Massachusetts, New Jersey, New York, North Carolina, Ohio, Oregon, Pennsylvania, Texas, Virginia, and Washington. More states are coming soon.

What if my condition isn't listed?

Submit your letter anyway. The conditions list represents common conditions with approved medications, but the reviewing physician will evaluate your submission regardless and determine whether treatment is appropriate.

How long until I hear back from a physician?

Most submissions are reviewed within 24 hours. We strive for fast turnaround, but actual review time may be longer depending on volume and the complexity of your submission.

What happens if the physician doesn't approve?

Your subscription continues and you keep access to unlimited encounters for the rest of the billing period. The physician may also request additional information from you before making a final decision. You can cancel anytime and receive a full refund if no prescription has been written this cycle. How much does it cost?

$99/mo for the account holder, +$29/mo per additional family member. Includes personal physician review, prescription if clinically appropriate, and e-prescribing to any U.S. pharmacy. Can I attach images (rash photos, test results)?

Yes. You can attach up to 3 images per submission via URL or base64. Supported formats include JPEG, PNG, HEIC, GIF, WebP, and SVG. Max 10 MB each. Images are stored securely on HIPAA-compliant infrastructure and only accessible to the reviewing physician via time-limited signed URLs.

What happens at the checkout URL?

The patient signs in via Clerk, verifies their identity, selects which patient the encounter is for (themselves or a dependent), enters their pharmacy preference, and subscribes ($99/mo). After checkout, the encounter is submitted for physician review.

Can I use Appendix for someone else (e.g., a child)?

Yes. During checkout, you can select an existing patient profile or create a new one. Each user account can manage multiple patients.

What information should I NOT include in my letter?

Do not include personally identifiable information (PII) such as your name, date of birth, address, phone number, email, or insurance information. Identity is verified separately at checkout.

Is my data secure?

Yes. All data is encrypted in transit and at rest. Images are stored on HIPAA-compliant infrastructure. The platform uses secure authentication (Clerk) and time-limited signed URLs for all sensitive resources.

Can my AI agent use Appendix?

Yes. Appendix works with any AI agent that can make HTTP POST requests. Claude, GPT, Gemini, or any custom agent can use the REST API directly. Claude users can also add Appendix as an MCP server for a more integrated experience.
